Title: Hospice Center

Description: Hospice care is provided wherever the patient lives, including at home, or in extended care facilities or nursing homes. We also care for patients at The Hospice Center, an inpatient unit for patients with short-term special care in order to manage an episode of acute pain or symptoms, or for patients whose caregiver needs a short break (respite care). We care for patients with a team of professionals including: Physician, Registered Nurse, Social Worker, Chaplain and Volunteer. We also offer special services for therapies (physical, occupational, speech), dietary expertise and provide medical equipment and pharmacy related to the hospice diagnosis. Bereavement services are available to families for 13 months after their loved one passes and bereavement counseling and groups are available to anyone in the community.
